Instructions at the bottom of the main page mention an Email form that may not exist anymore?

I'm fighting a problem with an account not showing payed (see #78 and #79). The main page at the bottom indicates folks with this problem should contact you (Pete?) through an "email-form". I don't find one of those and suspect it's been supplanted by this lighthouse project. If so, please update the bottom of the main page to reflect that. If not, please let me know where the form is so I can send this issue through the channel you want me to.
